Try my revolutionary designed Referees' Match Pad insert card, constructed from a single piece of A4 standard sized paper using Microsoft Word. When cut and folded, this match pad will provide you with 7 folding pages for the Home Team and 7 pages for the Away team. FlagBase is a program designed for Football (Soccer) Referees to keep records of all aspects of their career on a database, with easy access to information when required.
What the program does & Operating Requirements.
The program can keep information about Clubs, Competitions, Events, Grounds, League Contacts, Misconduct Reports, Money, Organisations, Other Referees and most importantly Matches. The program also has report form templates for some County FA's in the South East, but the number is continually expanding. 
The program has easy to use information input forms and buttons to perform a magnitude of tasks. The program has a massive amount of pre programmed searches, and the ability to produce lists and/or directories at the touch of a button. Custom Searches and Lists can be designed by the individual. 
The program is built in Microsoft Access, and there are two versions: FlagBase in Microsoft Access 97 and FlagBase in Microsoft Access 2000 (and Higher). Unfortunately, the Access 97 version cannot guarantee all features of the program. However MS Access 97 or higher is essential. 
To make full use of the program, the user requires Microsoft Excel 97 or higher (NOTE that the Excel Supplements were built and used in Excel 2000). These supplements include a workbook to analyse your misconduct reports, showing various statistics.

This design can easily be placed on the back of a postcard, and enables you to accept (OK), decline(NO), provide open dates(OPEN) when you are available to accept future appointments, and to close(CLOSE) dates that you are not available to receive future appointments on, from your Referees' Appointments Secretary.

This is just another chore that the Referee could organise before he gets to the game - giving the Referee one less thing to think about.

Use these forms to inform your appointment secretary which dates you are available on. These are known in England as Open and Closed dates.  
OPEN dates, are those dates that you are available to Referee on. 
CLOSED dates, are those dates that you are not available to Referee on.

(a) Word Document Form, consisting of check boxes for you to complete for each day of the season. Will can complete this form using Microsoft  Word and send by email.
(a)  A pdf format version, that is very small in file size. You can complete this form in manuscript and send by post.
