SECTION 1 - PERSONAL DETAILS

• This is for you to complete your own details (note, this section must be completed) and confirm you have registered with your County Football Association for this season. Please complete this section on receipt of your logbook. Should you lose your logbook if found it can then be returned to you.

SECTION 2- FITNESS

• If you attend and are successful in a fitness test below contributory level you are now able to include this within your logbook. The higher the level of fitness you demonstrate the more points you will receive. The points you can claim are given in the logbook. (Please note, the test must be taken between the 1st Aug 2006- 31st July 2007)

SECTION 3- MATCH DETAILS

• This section is for you to complete the games you officiate either as a referee, appointed assistant referee, fourth official or assessor.
• Please ensure that one of the clubs signs the final column to verify you officiating the game, prior to the game commencing. By getting the Iogbooks signed before the game this will avoid the need to get the logbook signed afterwards in case it is a difficult game. If the majority of signatures have not been obtained from either clubs or competition organisers we will be unable to process your claim later in the season.
•  All games officiated in must be officially sanctioned competitions and leagues, not friendlies and only 11 a side football of all ages, genders and abilities will count.
• Remember the games covered by this scheme are only those at and below Supply League level.
• Do not rely on referees secretaries to sign your logbook at the end of the season. It is your responsibility to get your logbook signed before each game. (FAMOA will not contact referees secretaries on your behalf).
• If you have joined the scheme part way through the season it is your responsibility to get the games you have already done signed, if you officiate for the club/s again during the season you can get it signed when you revisit them. If this is not likely please ask an official from the league, your regional manager or a county representative to sign your logbook for you.
• The maximum number of games you can claim for is 60 in total, this can be made up of any combination of those as a referee, assistant referee, fourth official or assessor.
• Enter all games into your logbook, games on separate pieces of paper will not be accepted.

SECTION 4- REFEREE RECRUITMENT

• This section is for you to confirm the details of any referees you recruit this season who have registered and completed 10 matches, and received their record of achievement.
• You are required to complete the columns in full. To claim the points the individual you have recruited must have applied for and received the 10 Match Record of Achievement certificate from The FA.

SECTION 5- REFEREES MENTORED

• This section is for you to confirm the details of any registered referees for whom you act as a mentor.
• You are required to complete all the columns in full.

SECTION 6- ATTENDANCE AT REFEREE ASSOCIATION MEETINGS

• If you attend meetings of your local Referees Association Branch or Society you will receive points. You are required to get an officer at the society to sign your logbook to confirm your attendance.

SECTION 7-ATTENDANCE AT CONTINUATION TRAINING

• If you attend any continuation training organised by The FA, including FAMOA events, or County FA continuation training events (listed below), you can claim points as a reward. You will need to get one of the instructors involved to sign your logbook to confirm your attendance. For more information on any of the listed continuation training courses please contact your County FA.

SECTION 8 - INSTRUCTORS

• Qualified FA Instructors may now claim points for basic training courses they instruct, continuation training evenings presented and attendance at Instructors Development Days and Instructors Panel Meetings. Please complete all the relevant information and get your County Training Officer or Regional Manager to sign this section of your logbook at the end of the season.

Please complete each section accurately and retain the completed logbook with your points total until you receive your order form. The space in the logbook for each section is the maximum number of points you can claim for the section; additional claims on separate sheets will not be accepted.

Please note points earned cannot be carried over to next season and there will be a ceiling on the total points you can claim of 1500 for the season.

How do I spend my points?

You can use your points to purchase a variety of referees kit and Umbro merchandise.

You will receive the order form early in the new year.

Finally, please remember to keep your completed logbook until you receive the order form later in the season.
